Fairview Health Services is seeking a Complex Care Physician in an outpatient setting at our M Health Fairview Clinics and Surgery Center Maple Grove location to help with growth in our complex care department. W e are looking for a provider who is willing and able to help us determine best practices and participate in a learning model to help provide the best care possible for our complex patients.
Our team is focused on prevention of hospitalization and increased access to and integration with home care . Work alongside RNs, clinical pharmacists, and other team members dedicated to treatment of these complex patients. The clinic has great access to more than 30 specialties including cancer care, podiatry, cardiology, dermatology, endocrinology, diabetes education, behavioral health and more. The clinic also has echocardiography, general and advanced imaging, and mammography and a complex laboratory on site.
Patient encounters average 10 patients per day.
Openings: 1.0 FTE (Full Time)
36 patient contact hours per week. Preferred schedule to be Monday through Friday.
No call required.

BC/BE with the ABMS or AOA.
Licensed or ability to obtain licensure through the Minnesota Board of Medical Practice.
Ability to obtain and /or maintain DEA certification for the State of Minnesota.
