VA Western Colorado is seeking a full time Geriatrics Extended Care (GEC) physician to serve as a Medical Director of Home Based Primary Care (HBPC), and Palliative Care Programs. VA Western Colorado Health Care System is one of the smaller VA facilities in the nation yet the geriatrics division encompasses a broad spectrum of programs which requires medical staff to fulfill a broader scope of responsibilities. The physician practices in the inpatient and outpatient Geriatrics Service Line settings in the WCHCS which may also include geriatric consultative support and attending physician for Residency program. The provider must be comfortable and capable of working in interdisciplinary teams to provide comprehensive patient care. The provider will also function as Medical Director over the Palliative Care Program and/or Home Based Primary Care Program. The provider is academically and clinically prepared to perform a broad spectrum of patient care services at the expert level. In addition to providing direct clinical care, the physician serves as a consultant to the interdisciplinary team for patients with complex treatment or care needs within GEC Services. Primary clinical duties include assessment, diagnosis, treatment and outcome measures of healthcare issues, histories and physicals, ordering diagnostic and therapeutic tests/studies, initiating consults and referrals, and prescribing medications/supplies in accordance with VA policy, VA formulary, Colorado state regulations and scope of practice.
